- Volunteer Form: Teams must provide parent/adult judges for our tournaments to run successfully. The Volunteer Form is required for parents to chaperone trips as drivers and lay judges. When members register to compete, parents are expected to judge. All participating parents must have Level 2 Volunteer clearance through the District. - School Club/Activity Approval and Student Registration Form: This mandatory form allows students to join a specific club. It must be signed and submitted to Mr. Froelich before attending meetings.
- Emergency Medical/Treatment Consent for Field Trips/After School Activities Form: This mandatory form allows supervising Pine View staff to properly contend with any medical situations that may arise on field trips.
- Release for Out-of-County or Overnight Travel for Athletics and Field Trips Form: This mandatory form allows students to participate in tournaments (all Speech and Debate tournaments are considered Field Trips). This form requires notarization.
- Private Vehicle Transportation Form: This mandatory form allows students to travel to tournaments in their parent's vehicle.
